Physical Therapist
Kind Rehabilitation provides mobile outpatient therapy services in the comfort of clients’ homes and community settings across Colorado. We contract with licensed clinicians who value independence and flexibility, while sharing our commitment to holistic, compassionate, 1-on-1 care. Our model is designed to give clients and their families meaningful, personalized therapy experiences, and to give contractors the freedom to practice in a way that feels both professional and person-centered. Patients present with a wide range of diagnoses, including orthopedic, cardiac, and both acute and progressive neurological conditions, and they are able to make meaningful progress through our treatments.
Position Summary
Kind Rehabilitation is seeking a licensed Physical Therapist (PT) who enjoys working closely with clients in a 1-on-1 setting. You set your own schedule, choose your treatment radius, and manage your own caseload, while having the security of an outpatient-style model that allows continuity of care without being encumbered by Medicare Part A rules, using outpatient-style planning and billing.
Responsibilities
Provide physical therapy evaluations and 1-on-1 treatments in home and community settings
Develop and carry out individualized care plans using outpatient-style planning and billing
Educate clients and caregivers on exercise programs and self-management strategies
Keep timely and accurate documentation in compliance with state and regulatory requirements
Communicate directly with clients and, when appropriate, their healthcare providers
Manage your own caseload, schedule, and travel arrangements
Qualifications
Current Colorado Physical Therapy License in good standing
Graduate of an accredited Physical Therapy program (DPT or MPT)
1+ years of clinical experience preferred (outpatient, home health, or geriatrics)
Reliable transportation and ability to travel within designated service areas
Professional liability insurance (required for contractors)
Strong clinical and communication skills
Comfortable working independently and managing your own workload
Contract Details
Independent Contractor (1099) status – not an employee role
Competitive per-visit rates (negotiated in contract)
Flexible scheduling – you choose your own availability and referrals
Contractors are responsible for their own taxes, insurance, and licensing requirements

Develop and carry out individualized care plans using outpatient-style planning and billing
Educate clients and caregivers on communication, swallowing, or cognitive-linguistic strategies
Keep timely and accurate documentation in compliance with state and regulatory requirements
Communicate directly with clients and, when appropriate, their healthcare providers
Manage your own caseload, schedule, and travel arrangements
Qualifications
Current Colorado Speech-Language Pathology License in good standing; CCC-SLP preferred
Graduate of an accredited Speech-Language Pathology program
1+ years of clinical experience preferred (outpatient, home health, or geriatrics)
